To change your name please complete the Change of personal details form.

Please send us the completed form along with the relevant documents, including proof, such as your marriage certificate. You can send the original documents or certified copies.  We'll return the original documents to you and the certified copies if you ask for them, as soon as we can.

If you have a product through your employer, please contact them to update your name.
